We provide individualized services through every stage of life to build independence, strengthen daily living skills and support greater participation at home and in the community. From in-home and day programs to residential services, our approach is rooted in dignity, respect and strong partnerships with families.

Pat Lebon Adult Day Treatment Center

The Pat Labon Center in Lumberton, NJ helps individuals living with developmental and/or intellectual disabilities build skills, develop relationships, explore interests and participate more fully in community life.

Community Group Homes

For children and adults living with developmental and/or intellectual disabilities who need around-the-clock support, we provide 24/7 residential care in safe, accessible community-based homes.

Services are individualized and may include therapy, medical care, behavioral support, daily living skills development and opportunities for socialization, recreation and community involvement.

Supported Living

For adults living with developmental and/or intellectual disabilities who are ready to live more independently, supported living offers the opportunity to live in the community with the right level of assistance.

Support may include transportation, nutrition, shopping, budgeting, medication administration and community involvement. Staff are available 24 hours a day, 7 days a week when support is needed.

Employment & Vocational Support

Meaningful work can provide confidence and a greater sense of independence. Employment services help individuals explore strengths and interests, develop workplace skills and pursue opportunities that fit their goals.

In-Home Services: Bridging Family Support

In-home support for people with intellectual and developmental disabilities and their families.

Bridging Family Support provides intensive in-home services for people of all ages who experience communication, social interaction or behavioral challenges. Support may include therapy, daily living skills, behavior management and respite care.

Family & Caregiver Support

We help families understand available services, navigate transitions and connect with resources that support both the individual and those who care for them.

Transitioning to Adult Services

Turning 21 can bring a lot of questions about what comes next.

Our team helps families navigate the transition from children’s services to adult developmental disability services, including day programs, supported living and 24/7 residential support.

Our goal is to help make the process clearer, connect families with the right resources and support continuity of care as needs change.

How to Access Services

For Children and Families:
Oaks works closely with PerformCare for children under age 21. Families can contact PerformCare to determine eligibility and begin the referral process.

For Adults:
Adult developmental disability services are coordinated through the New Jersey Department of Developmental Disabilities (DDD). Families should work with their identified DDD Support Coordinator (case manager) to determine eligibility, enroll in Medicaid and explore available services.
