When to Reach Out

You may want to reach out if you or someone you care about:

  • Is having difficulty navigating services or resources
  • Needs ongoing support to remain stable in the community
  • Is moving from one level of care or life stage to another
  • Needs help building independence or everyday life skills
  • Is involved with the criminal justice system and needs community-based support

Every service has its own eligibility requirements and referral process. Our team can help you determine which options may fit your needs.

Find the Right Fit

Support is based on what is happening in your life and what you need to move forward. That may mean connecting with treatment, learning new skills, finding practical resources, navigating a transition or having someone help you understand the options available.

Case Management

Living with a serious and persistent mental illness and/or substance use disorder, can make it harder to manage everyday needs and maintain stability in the community. Intensive case management provides ongoing support to help adults navigate those challenges, reduce the need for hospitalization and improve quality of life.

Support may include engagement and assessment, individualized service planning, connections to treatment and community resources, system advocacy and care coordination.

Justice-Involved Services

For someone experiencing mental health challenges, involvement with the justice system can quickly create additional barriers.

Through jail diversion and correctional case management, our team works with law enforcement, including Crisis Intervention Team (CIT) trained officers, to help divert non-violent individuals with mental health needs from incarceration and connect them with community-based services.

Support may include case management, transition and reentry planning, connections to mental health and substance use treatment, and help accessing housing, benefits and other community resources.

Family & Caregiver Support

Supporting someone you care about can come with a lot of questions.

Mental health challenges often affect the entire family. Intensive Family Support Services (IFSS) provides education, guidance and support for family members and caregivers. Available resources include advocacy, support groups, social activities, referrals and more.
